New Job Invocation page shows duplicate dropdown “Job template *”

1) In “Job category” select one of Power, Puppet

2) “Job template” rendered twice:
  • Power Action - Ansible Default
  • Power Action - SSH Default
“action” rendered twice:
  • restart
  • restart

“puppet_options” rendered twice
