Dominic pointed out that calling directly to endpoints is now deprecated in favor of `:rackspace_region`. We'll need to support their private cloud offerering, too, so the form should allow for an "other" (or something more descriptive than that) region which then exposes a text field to enter the private cloud endpoint.
