Extend the prototype screen with real code with this workflow:

• redo the prototype screen with checkboxes and nicer TUI
• if proxy.url and proxy.type kernel options were provided, fill them
• if SRV DNS (_x-foreman._tcp) record was found, fill it in
• the same for newly created _x-foreman_type._tcp
• if none found, "foreman" and type of "proxy" should be the defaults
• allow to test the connection (new button)
• prepare the screen for login/password (hidden for now)
