If there's a validation problem with the selection of organizations/locations, the form does not show any error message. If that's the only issue in the form, user has no clue why the record does not save or what happened. To reproduce

1. as non-admin user with organizations assignment permissions for at least two orgs, (you can use Organization admin role assigned to two orgs)
2. try to create a domain but do not select any organization, fill all other fields
3. see the form, there's no error, the failure is only logged

you should see red Organizations tab with error message below the selector
