Ability to choose disk type and assign name to disk while provisioning.

Description
When provisioning new VM in RHEV/Vmware/Libvirt using a compute profile which has defined virtual disks, we cannot choose which disk type will be used (VirtIO / VirtIO-SCSI) nor choose which name will be assigned to the disk.
Need a kind of text input / dropdown to choose those options on host creation time.
