Host bread crumb inconsistency: uses ID but url uses host's name on first visit
Description
The hosts breadcrumb currently uses the host id however in the url the name of the host is used when first visiting a host from the hosts index.
We should change the host breadcrumb to use the name of the host instead.
Steps to reproduce:
1. Visit hosts index.
2. Click on a Host; take note how the host name is in the url.
3. Click on hosts bread crumb and select a different host.
4. Take note of how the URL now contains the numerical ID of the host, not the name.
