Description

As a foreman user I want to add a comment when I disable a notification using the Web UI and the REST-API.

The comment should be presented in the host's info page and GETtable via the REST-API.

We're regularly use the "disable notification" feature on Foreman to allow a host with a temporarily disabled Puppet agent or hosts in maintenance. An optional comment field would make communicating about why one disabled a host easier.
