Unable to save
Failed to create newhost.example.com's realm entry: ERF42-5349 [Foreman::Exception]: Realm proxy did not return a one-time password

Check the FreeIPA logs for the reason why it didn't return a one-time password (generally, /var/log/httpd/*error_log). This may because the host already exists in FreeIPA, or your user doesn't have the right permissions to create a new host.
