This happened to me today:

[kdevel@foretello foreman]$ rake db:migrate
rake aborted!
ArgumentError: Key: 'url_prefix' in 'development' environment should be one of ["/headpin", "/sam", "/katello"], but was nil
[...]

As it turned out, url_prefix was indeed not set.
