Description
Foreman-proxy server fails to start on discovery image 3.1.2. Output from discovery-debug is below. When I start foreman-proxy manually after a while, it start without issue.
Problem is probably caused by the fact that server has many NICs and DHCP takes longer and network is not prepared on time, but this is only a guess.

Kamil, known issue: #15195 and this looks like it (45 secs is default in NM and you booted exactly the same time).
Unfortunately there is no workaround until we rebase onto CentOS 7.3 which will allow the DHCP timeout setting in NM. See the ticket and https://github.com/theforeman/foreman-discovery-image/pull/75 for more info.
I am closing this as a dupe, feel free to reopen if you find something.
